Please note, all forms are required regardless of applicability. For example: the Medication Waiver form is required even if your pet is not currently on any medications. A situation/illness may arise while they are in care where veterinary services may be necessary and medications are needed. (i.e ear infection, UTI, vomiting/diarrhea)
